Hospice Eligibility Criteria for HIV & AIDS

 Caring for a patient with acquired immunodeficiency syndrome (AIDS) or an HIV infection comes with specific challenges. Because human immunodeficiency virus (HIV) affects the immune system, advanced forms of this disease often involve concurrent illnesses. This can complicate care because the symptoms of terminal-stage HIV may include everything from renal failure to mental confusion. 

Deciding When to Start Hospice Care for Patients With HIV

Patients typically enter hospice after curative and life-sustaining treatments have been exhausted. Hospice prioritizes quality of life and symptom relief, ensuring patients spend their final days, weeks, or months with dignity and comfort.

While physicians usually refer patients to hospice following a terminal diagnosis, patients and families can also initiate the conversation. Planning ahead makes the transition smoother when the time is right.

Eligibility for hospice is based on various factors, including disease progression and overall health status—not just specific lab results. For individuals with HIV/AIDS who are not yet eligible for hospice, palliative care may be a suitable option. It offers many of the same supportive services while allowing continued treatment for the underlying illness and is ideal for patients with a life expectancy greater than six months.

Diagnostic Criteria for Entering Hospice Care with Advanced HIV Disease

 

The diagnostic criteria for hospice admission include a life expectancy of six months or less and the decision to stop curative treatments. For patients with AIDS or HIV, determining eligibility can be more complex due to co-occurring conditions that impact symptom progression and treatment response. Medical guidelines for terminal status in HIV/AIDS involve a combination of clinical assessments and documentation of specific qualifying symptoms.


Comorbid conditions that may support a terminal HIV/AIDS diagnosis include:

  • Central nervous system (CNS) lymphoma
     
  • Progressive multifocal leukoencephalopathy (PML)
     
  • Visceral Kaposi’s sarcoma unresponsive to therapy
     
  • Systemic lymphoma unresponsive or only partially responsive to chemotherapy
     
  • Mycobacterium avium complex (MAC) bacteremia unresponsive or untreated
     
  • Severe wasting syndrome (loss of 33% or more of lean body mass)
     
  • Discontinuation or failure of antiretroviral therapy (ART)
     
  • End-stage renal failure
     
  • Untreatable toxoplasmosis
     
  • Cryptosporidium infection
     
  • Cytomegalovirus (CMV) infection
     

Primary hospice eligibility criteria for patients with HIV/AIDS include:

  • A significantly decreased functional status, typically measured by a Karnofsky Performance Scale (KPS) score of 50% or less
     
  • A CD4 count below 25 or a persistent viral load greater than 100,000 copies/mL, along with at least one of the listed comorbid conditions
     

Additional factors that may support hospice eligibility:

  • Age over 50
     
  • Advanced AIDS dementia complex
     
  • Congestive heart failure
     
  • Chronic diarrhea
     
  • Persistent serum albumin of 2.5 g/dL or lower
     
  • History of substance use disorder
