Hospice for Renal Disease

Patients with end-stage renal disease (ESRD) require specialized support to ensure comfort and dignity in their final days, weeks, or months. Elation Hospice & Palliative Care provides comprehensive care focused on managing symptoms, offering emotional support, assisting with daily activities, and easing the burden on family caregivers.

Hospice should not be seen as giving up, but rather as a compassionate choice to enhance quality of life during the final stages of kidney disease. The goal is not to hasten death, but to provide comfort, peace, and support for both the patient and their loved ones.

Hospice Criteria for Renal Disease Patients

Kidney disease impairs the body’s ability to filter waste, regulate blood pressure, and manage fluid balance. In end-stage kidney disease, the kidneys fail completely, leading to toxin buildup and eventual organ failure.

Elation Hospice & Palliative Care provides specialized support for patients with end-stage renal disease, focusing on comfort, symptom relief, and quality of life.

Patients may qualify for hospice care if they:

  • Are not eligible for or have chosen to stop dialysis or kidney transplant

  • Have a creatinine clearance of ≥8.0 mg/dL (or ≥6.0 mg/dL with diabetes)

  • Are over 60 with coexisting conditions such as heart disease, COPD, cancer, or liver disease

Hospice is typically appropriate for patients with a prognosis of six months or less who have stopped curative treatment. Those continuing dialysis may still be eligible for palliative care, which focuses on symptom management and support while continuing active treatment.

Choosing Hospice for Renal Disease

Deciding when to begin hospice care is a collaborative process involving the patient, caregivers, and healthcare providers. Caregivers may include spouses, adult children, or close family friends. While referrals are often made by a primary care physician, both patients and caregivers can request one.

If hospice criteria aren’t yet met, palliative care may be a suitable alternative. It allows kidney disease patients to continue treatments like dialysis while receiving support for symptom management and daily needs. Palliative care is also appropriate for those with a longer life expectancy who need more help than loved ones can provide.

Planning ahead—before kidney disease reaches the final stage—ensures care is in place when needed. By understanding the progression of symptoms, families can determine the right time to transition to hospice care.

Patients with chronic kidney disease who receive hospice support often avoid unnecessary hospitalizations and experience greater comfort, while caregivers benefit from reduced stress and guidance from a compassionate care team. Elation Hospice & Palliative Care is here to support you every step of the way.

How Hospice Care Helps Patients with End-Stage Renal Disease

When a patient with end-stage kidney disease chooses hospice care, the goal is to enhance quality of life and provide relief from pain and other distressing symptoms. The process begins with a comprehensive hospice evaluation to determine the patient’s specific needs and goals—such as managing kidney failure symptoms or remaining at home with loved ones.

Care plans are fully personalized, and each member of the hospice team is selected based on the patient’s unique preferences and medical needs, ensuring comfort, dignity, and support throughout the end-of-life journey.

Compassionate Hospice Care Services

Symptom Management

Patients with end-stage kidney failure often experience a range of symptoms, including pain, fatigue, nausea, loss of appetite, difficulty sleeping, itching, and emotional distress. If other organs are affected, discomfort may become more widespread and complex.

Elation Hospice & Palliative Care focuses on comprehensive symptom management to maximize comfort and quality of life. Pain relief may include prescribed medications as well as alternative approaches tailored to the patient’s needs. The hospice team works closely with the patient’s primary care physician to monitor symptoms and adjust the care plan as needed.

Nutrition specialists help create personalized meal plans, while nurses provide wound care and other supportive services. Every aspect of care is designed to ease suffering and support the patient physically, emotionally, and spiritually.

Coordinated Care

Coordinated hospice care is built on collaboration among the patient, family caregivers, primary care physician, and the hospice team. This team may include nurses, counselors, physical therapists, social workers, chaplains, and trained volunteers—each playing a role in supporting the patient’s comfort and well-being.

Visit frequency is based on the patient’s condition and needs, ranging from daily support to weekly check-ins. Every care plan is personalized, with the patient’s wishes guiding the approach—whether that means staying at home, minimizing the burden on loved ones, or focusing on specific end-of-life goals. The result is care that honors the patient’s dignity, preferences, and priorities.

Personal Care

Emotional and Spiritual Support

As renal disease advances and physical strength declines, everyday tasks like bathing, grooming, meal preparation, and using the restroom can become increasingly difficult. Hospice nurses, home care aides, and trained volunteers provide hands-on support with these daily activities, helping patients maintain dignity, comfort, and a better quality of life throughout their care journey.

Emotional and Spiritual Support

Emotional and Spiritual Support

Hospice patients often benefit from additional emotional, mental, and spiritual support. Counselors and chaplains offer compassionate care through one-on-one sessions, helping patients process end-of-life concerns, manage anxiety or depression, and find peace through spiritual guidance—ensuring holistic care that nurtures both mind and spirit.

Medical Supply Assistance

Medical Supply Assistance

During the initial hospice assessment, the hospice care coordinator evaluates the patient’s needs and identifies the medical supplies and equipment required for ongoing care. The hospice team arranges delivery and setup of any necessary equipment, while also providing essential supplies—such as bed pads, gloves, and other items—to ensure safe, comfortable, and uninterrupted care at home.

In-Home Care

Hospice services are most often delivered wherever the patient resides—whether that’s their own home, a loved one’s home, or an assisted living facility—ensuring comfort and familiarity during end-of-life care.

Patients have access to care 24 hours a day through Elation Hospice & Palliative Care, with nurses and other team members available on-call to respond whenever support is needed. Regular visits from the hospice team ensure consistent, dependable care—providing peace of mind for both patients and their families.
