Hospice care from Elation Hospice & Palliative Care focuses on enhancing quality of life and providing expert symptom management for individuals facing terminal illnesses. A dedicated team—including nurses, social workers, hospice aides, volunteers, and other specialists—works together to relieve pain, offer emotional support, provide companionship, and ease the burden on family caregivers.
Care is physician-directed and personalized to meet each patient’s unique needs. Services are delivered wherever the patient resides—whether in their own home, a family member’s home, or a senior living facility—allowing them to remain in a familiar, comforting environment surrounded by love. Support includes help with daily activities, symptom control, and emotional relief, all designed to bring peace and dignity throughout the journey.

For individuals living with a serious illness while continuing treatment, Elation Hospice & Palliative Care offers palliative services that focus on improving quality of life at any stage of the diagnosis. Our palliative care team works in collaboration with your physician to provide relief from symptoms, reduce stress, and offer emotional and practical support for both patients and their families.
Palliative care is flexible and evolves as your condition or needs change. Some patients may transition between palliative and hospice care over the course of their illness, depending on the diagnosis, personal goals, and the anticipated outcome. All care decisions are made with input from the patient and medical providers to ensure a smooth, personalized care experience every step of the way.
